Interviews will be in-person only, and you must own a valid driver’s license or state ID. To respect the safety of our patients and employees, we perform a comprehensive criminal background check during the hiring process. We also prefer that you have at least one year of senior healthcare experience.

Job Description

  • Demonstrates complete competence in technical and manual skills according to the scope of practice for LPNs/LVNs.
  • Functions as a leader for the Care Partners; delegates in a manner that contributes to the excellence of the ARCP.
  • Administers medications and follows the seven (7) medications correctly. Reduces the potential for medication errors by carefully assessing resident’s conditions and nursing needs.
  • Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques.  Educates the resident and family regarding pain management.
  • Ability to interpret results of waived tests and take appropriate action on waived tests results.
  • Performs all aspects of resident care in an environment that optimizes the resident’s safety reducing the likelihood of medical/health care errors.
  • Meets current documentation standards and policies.
  • Notifies physician of changes in a resident’s condition and follows through until appropriate action is taken.
  • Notes and carries out physician’s orders promptly.
  • Gives thorough verbal reports to oncoming shifts.
  • Follows through as needed on the information given by shift report, resident, or family regarding resident concerns.
  • Functions as a resident advocate by protecting the resident’s rights.
  • Interacts with residents, families, co-workers, managers, and other departmental staff in a kind, knowledgeable, and friendly manner.
  • Consistently involves resident and family in planning for care, treatment, and teaching needs.
  • Demonstrates the ability to remain calm and in control in an emergency and/or stressful situation while exuding a sense of confidence.
  • Consistently follows facility policies and procedures.

Job Description

  • The primary purpose of our Certified Nursing Assistant (C.N.A.) Care Partner position is to assist elders in their routine needs and daily activities
  • Assist residents with ADLs (bathing, dressing, grooming, etc.).  
  • Assist residents with IADLs.
  • Ability to prepare residents for activities in the community, including medical appointments.
  • Helps prepare residents for meals and snacks and performs after-meal care.
  • Provides basic room care.
  • Supports and maintains a culture of safety and quality.
  • Ability to observe and report the presence of skin breakdowns.
  • Reports all accidents and incidents immediately to their supervisor.
  • Notifies the nurse of changes in the resident’s condition and follows through until appropriate action is taken.
  • Reviews task/assignment sheets daily and gives a thorough verbal report to the oncoming shift.
  • Treats residents and their families with respect and dignity.  
  • Interacts with residents, families, co-workers, managers, and other departmental staff in a kind, knowledgeable, and friendly manner.
  • Reports to work on time and as scheduled. Completes work within the designated time.
  • Completes in-service and returns in a timely fashion.
  • Represents the organization positively and professionally.

Qualifications:

  • 1-year experience (preferred)
  • Pass background check
